As some examples are good, loads just make the page to long and nobody can be bothered to scroll down and order xD

If you mean his application form, no but it shouldn't matter. And also, its not the exact same, it is different. Its not like I stole his art, and gave myself credit, Its an APPLICATION FORM. NO big deal.

SDB- Ill get started. BTW color scheme is like "wavy" or "shiny" I can make the backgroung yellow&red if you want. If you want a specific color scheme, please post it.
